Southwest System of Care Schools


The Southwest System of Care partners with six local elementary schools to connect families to resources on the southwest side of Chicago. Partner schools work in close collaboration with SWSOC Navigators to identity, refer and follow up with families connected to the SWSOC. The relationships built in our partner schools are essential to connecting families with the resources they need to be successful. Currently, schools in partnership with the SWSOC are Claremont STEM Academy, Eberhart Elementary School, Edwards Dual Language Fine and Performing Arts IB School, Hurley Fine and Performing Arts Magnet School, Marquette Elementary School of Excellence and McKay Elementary School.

How does a school become a part of the Southwest System of Care?

As funds become available, the Southwest System of Care opens a Request for Proposal to schools in our neighborhood. Schools are asked to submit a proposal outlining the ways in which they plan to integrate the supports and services of the SWSOC with their existing school systems. It is critical that schools' communication systems and student and family support systems align with the resources and supports of the SWSOC. The SWSOC seeks to become a part of the fabric of the schools with which it partners to support whole children, whole families and the whole community.
